Too often patients with liver failure remain under care of local providers who do not understand care of the advance liver failure patient. Frequently, patients with end stage liver disease are admitted to local hospitals and then transitioned to end of life care, without any formal consideration of liver transplant. Through focused outreach to community hospital intensivists and hospitalists, specialists at University of Utah Health are increasing the referral of patients at their most acute period for transplant evaluation.
